Most testosterone is attached to proteins such as SHBG (sex hormone-binding globulin) and albumin, meaning that free testosterone levels may decrease even when total testosterone appears normal - especially with aging, stress, certain illnesses, or changes in SHBG levels. Measuring both fractions provides a more accurate assessment of hormonal balance and helps determine whether symptoms may be related to testosterone deficiency.
